Student Athletic Health Forms

Welcome to the Student-Athlete Health Form Page!

In order for a Student-Athlete to participate in High School or Middle School sports, they must be fully cleared by the school doctor.  In order to do so, there are 2 sets of Student-Athlete Health Forms that must be submitted.

Set #1: the Athletic Physical and the Health History Update.
These two forms must be handed in to the school nurse in HARD COPY format.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.
