Sports Registration for Grades 6-12

Midland Park Athletic Teams

High School:
Fall: Soccer, Football, Girls Tennis, Cross Country, Cheerleading, Volleyball
Winter: Boys Basketball, Girls Basketball, Wrestling, Indoor Track, Bowling, Winter Cheer
Spring: Baseball, Softball, Golf, Spring Track, Boys Tennis


Middle School
:
Fall: Soccer, Volleyball, Cross Country (6th-8th)

Spring: Baseball, Softball, Spring Track (6th-8th)

Please read thoroughly for info on the Midland Park Athletics Registration process and follow the steps below. Doing so will ensure that your child is cleared to participate in time for the start of each season.  Students must submit appropriate paperwork to the nurse prior to participation.

Step 1:  Submit the electronic forms online.

  • The online submission website is open.
  •  All athletes must submit online forms first, BEFORE submitting physicals to the Nurse.
  • To submit forms electronically, click on the link: http://mphs.powermediallc.org
  • Next, click on "Sports Participation Packet" in the top right corner.
  • Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.
  • Finally, follow the prompts to complete all the necessary forms electronically.

Step 2: Submit a New Physical or Health History Update.

  • Students must have first completed their online forms before submitting Hard-Copy forms!
  • Once online forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.
  • The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in. State Athletic Physical Form
  • The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed. Health History Update Form

 In order for a Student-Athlete to participate in High School or Middle School sports, they must be fully cleared.  In order to do so, there are two sets of Student-Athlete Health Forms that must be submitted. Please note, online forms and registration must be completed before submitting paperwork to the nurse.


#1: The Electronic Clearance Forms.
These forms include:
(a) Parent Consent Form
(b) Steroid Testing Form
(c) Concussion Policy Form
(d) Sudden Cardiac Arrest Pamphlet sign-off
(e) Opioid Drug Use Pamphlet sign-off.


These 5 forms MUST BE SUBMITTED EVERY SEASON.  As of the 2016-2017 athletic season, we are requiring all athletes/parents to submit these 5 forms electronically
Doing so is very simple, just follow these easy steps:

According to Chapter 16 of the New Jersey Administrative Code Title 6A adopted by the State Department of Education, an annual sports physical form must be completed.  Please click on the following link for further information.  IMPORTANT NOTICE ABOUT ATHLETIC PHYSICALS


#2: the Athletic Physical and the Health History Update.

These two forms must be handed in to the school nurse in HARD COPY format.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

All physical forms and consent forms are to be returned to the school nurse.
Paperwork should not be submitted to the Athletic Director, or any coach or trainer.
