ANNOUNCEMENTS & UPDATES:

**All Technical Difficulties have now been resolved regarding the Online Athletic Submission Page.  Please follow the steps below once again in order to submit forms.  Sorry again for any inconvenience.**


High School (grades 9-12):

*Winter Sports sign-ups and clearance have begun!  See below for more information.
*Girls Soccer State Tournament begins October 31st vs Wood-Ridge, 2pm @ Midland Park
*Boys Soccer State Tournament begins October 31st vs Pompton Lakes, 2pm @ Pompton Lakes
*Volleyball advances to the 2nd round.  Game will be played on November 2nd.  Opponent and time are TBA.



Middle School (grades 7&8):

*Winter Sports sign-ups and clearance have begun!  See below for more information.





Winter Sports Clearance Information
*Winter Sports Registration has officially begun.  Please follow the steps below to insure your child is cleared to participate in winter sports starting in November 20th for High School and December 4th for Middle School.

Step 1:  Submit the electronic forms online.
   *The online submission website reopen for the winter season. When logging in, please be sure to select WINTER
   *All athletes must submit online forms first, before submitting physicals to the Nurse.
   *To submit forms electronically,
 click on the link: http://mphs.powermediallc.org

   *Next, click on "Sports Participation Packet" in the top right corner.
   *Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online last year.
   *Finally, follow the prompts to complete all the necessary forms electronically

Step 2: Submit a New Physical or Health History Update.
   *Students must have first completed their online forms before submitting Hard-Copy forms!
   *Once on-line forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.
   *If an athlete's most current physical expires before November 20th, 2017 they will need a new physical.
   *If an athlete's most current physical DOES NOT expire before November 20th, 2017, they will need a Health History Update.



The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

PLEASE NOTE:
*Winter High School Sports start November 20th.  Students WILL NOT be allowed to participate, no matter the issue, if they are not fully cleared.

*Fall Middle School Sports start on or around December 4th.  There may be tryouts, in which athletes must be fully cleared before participating in tryouts.
