*MS Baseball Tryouts Schedule:
Day 1 - Thursday, March 23rd, 3:15pm-4:30pm
Day 2 - Friday, March 24th, 3:15pm-5:00pm
Athletes should be prepared for both indoor and outdoor tryouts.
Spring Sports Clearance Information
*Spring Sports Registration has officially begun. All physical clearance forms, EXCEPT FOR THE STATE PHYSICAL FORM or HEALTH HISTORY UPDATE, are to be completed on-line using our new electronic submission form.
*The Online Submission Website is now OPEN!
To submit forms, click on the link: http://mphs.powermediallc.org
-Next, click on "Sports Participation Packet" in the top right corner.
-Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online last year.
-Finally, follow the prompts to complete all the necessary forms electronically
*Once on-line forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update (if the athlete's physical is up-to-date).
The Athletic Physical Form (found in the link below) must be newly completed every year. The Athletic Physical Form is good for 365 days. When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

Spring High School Sports start on March 3rd, and
Spring Middle School Sports start on March 20th, so be sure to complete your forms asap.
