High School (grades 9-12):

*Varsity Girls Soccer game for today (9/25) is postponed.
*Varsity Boys Soccer game for Wednesday, September 26th has been moved to away, at Palisades Park.

Middle School (grades 7&8):

*MS Girls Soccer game for today (9/25) has been postponed.
*MS Boys Soccer game for today (9/25) has been postponed.

Fall Sports Clearance Information
*Fall Sports Registration will officially begin July 2, 2018.  Please follow the steps below to insure your child is cleared to participate in fall sports starting on August 6th/13th for High School and September 7th for Middle School.

Step 1:  Submit the electronic forms online.
   *The online submission website will reopen on July 2nd for the fall season. When logging in, please be sure to select FALL
   *All athletes must submit online forms first, before submitting physicals to the Nurse.
   *To submit forms electronically,
 click on the link: http://mphs.powermediallc.org

   *Next, click on "Sports Participation Packet" in the top right corner.
   *Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online last year.
   *Finally, follow the prompts to complete all the necessary forms electronically

Step 2: Submit a New Physical or Health History Update.
   *Students must have first completed their online forms before submitting Hard-Copy forms!
   *Once on-line forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.
   *If an athlete's most current physical expires before August 6th, 2018 they will need a new physical.
   *If an athlete's most current physical DOES NOT expire before August 6th, 2018, they will need a Health History Update.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

*Fall High School Sports start August 6th for Football and August 13th for the rest.  Students WILL NOT be allowed to participate, no matter the issue, if they are not fully cleared.

*Fall Middle School Sports start on September 7th.  There may be tryouts, in which athletes must be fully cleared before participating in tryouts.
