Winter Sports Registration for Grades 7-12

Below are the instructions for winter sports registration. Students must submit appropriate paperwork to the nurse prior to participation.  

High School Winter Seasons

Season 2 - Basketball, Bowling: January 11 (Practice) & January 26 (Competition)

Season 2a - Winter Track & Field: February 1 (Practice) & February 16 (Competition)

Season 3 - Girls Volleyball, Wrestling: March 1 (Practice) & March 16 (Competition)

Cheerleading: January 18

Middle School Winter Seasons

Middle School Basketball (Boys/Girls): Practice/Tryouts begin January 20

All other Middle School Winter Sports: TBD

Please note that registration is open for all of the sports listed above. Please follow the instructions below for registration. 

Attention Parents and Student-Athletes:

Winter Sports Registration still continues.  Please follow the steps below or visit the Athletics’ Webpage for similar information.  Doing so will ensure that your child is cleared to participate in time for winter sports starting on the dates listed above

Step 1:  Submit the electronic forms online.

The online submission website is open (as of November 17, 2020).

All athletes must submit online forms first, BEFORE submitting physicals to the Nurse.

To submit forms electronically, click on the link: http://mphs.powermediallc.org

Next, click on "Sports Participation Packet" in the top right corner.

Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.

Finally, follow the prompts to complete all the necessary forms electronically.

Step 2: Submit a New Physical or Health History Update.

Students must have first completed their online forms before submitting Hard-Copy forms!

Once online forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

Dear Parents, Students and Midland Park Community:

 We are excited for the return of sports at Midland Park Jr./Sr. High School!  With the fall season set to begin in just a few days, we wanted to share with you important guidelines and protocols from the governor’s executive orders, the NJSIAA, the NJIC and our district. With health and safety being the first priority, the following guidelines will be in place for the 2020 fall season. Please understand that these guidelines and protocols are subject to change based on changes from athletic, health and state agencies. In that event, you will be notified during the course of the season. 

 Midland Park Jr./Sr. High School Fall Sports/Activities:

  • Soccer, Cross Country, Tennis, Football, Cheer, Marching Band


  • Spectators must wear face coverings and be socially distanced.
  • Bleacher seating for games at Midland Park’s Santorine Stadium Field will be marked off accordingly. Seating/standing arrangements for games/events where there are not bleachers must still follow social distancing and face covering guidelines. 
  • At this time, the NJSIAA guidelines and Governor Murphy’s executive order limit outdoor gatherings to no more than 500 people. Each host school will determine the maximum capacity for their event(s).  
  • Failure to comply with these rules will result in possible removal from the grounds at home and/or away events.

 Game Participants/Sideline Area

  • As per NJSIAA guidelines, players/participants not involved in play shall wear face coverings. 
  • Sidelines should be limited to active participants and personnel that are essential to the event.
  • To the largest extent possible, social distancing should occur on the sidelines. 

 Building Access/Locker Rooms/Concessions

  • During Midland Park athletic events, the school building will be closed. The locker rooms and bathrooms inside of the school will not be available. Portable bathrooms are located behind the stadium bleachers.
  • The stadium concession stand at Midland Park will not be available this year. 

 Football/Cheer/Marching Band

 Please understand that these guidelines and protocols are new to all of us and we ask for everyone’s cooperation so that we may enjoy our fall sports season and provide the best possible environment for our students, families and spectators.

     *The NJSIAA has recently published its most up-to-date version of the Return to Play Plan '20-'21.  Please note that this document is an ever changing document, and will be continuously updated.
