Midland Park now accepts the PPE Medical Eligibility Form and the Health History Update Form in person or via email. Email forms to [email protected]. Attach the forms to a blank email and include the student’s first name, last name, and sport in the subject line.
If you would like to submit either the PPE Medical Eligibility Form or Health History Update Form in person our school nurse has the following summer hours.
