Sports Registration & Athletic Clearance

Please read thoroughly for info on the Midland Park Athletics Registration process and follow the steps below. Doing so will ensure that your child is cleared to participate in time for the start of each season.  Students must submit appropriate paperwork to the nurse prior to participation. 

Step 1:  Online Sport Registration

All athletes must register and submit online forms first, BEFORE submitting PPE Medical Eligibility forms to the Nurse.

  • Next, click on "Sports Participation Packet" in the top right corner.
  • Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.
  • Finally, follow the prompts to complete all the necessary forms electronically.
  • Health History Update Form is now integrated into the Online Sports Registration Process, eliminating the need to submit it as a separate document. When registering your student-athlete for a sport, you will complete the Health History Update within the online platform, ensuring a streamlined and efficient process.

These forms must be submitted electronically every athletic season (Fall, Winter, Spring, Summer).

If your child’s physical is up-to-date, meaning it was completed within 365 days of the season start date (High School 11/25/24 & Middle School 1/6/25), and is already on file with the nurse, you’re all set—no need to move on to step two or submit any additional paperwork. If the physical is outside this 365-day window, your child will need new medical clearance, so please proceed to step two to submit the updated PPE Medical Eligibility Form.

Step 2: Medical Clearance

For students who still need medical clearance, once the electronic registration is complete, you must submit the state-mandated PPE Medical Eligibility Form to the school nurse. Please read below.

  • Athletic Pre-Participation Physical Examination Forms are still required and are valid for 365 days from the exam date. The physical must be valid at the start of each season. If the current physical expires before the beginning of practice, a new physical is required. However, please note that this form is no longer submitted to the school and should instead be kept on file with your student-athlete’s primary care physician.
  • The new PPE Medical Eligibility Form is now the only document submitted to the school nurse after completing a new physical. This form must be filled out by your student-athlete’s doctor and then returned directly to the school nurse to confirm medical eligibility for sports participation. (Any Physicals Completed after 6/30/24)
  • Health History Update Form is now integrated into the Online Sports Registration Process, eliminating the need to submit it as a separate document. When registering your student-athlete for a sport, you will complete the Health History Update within the online platform, ensuring a streamlined and efficient process.


Midland Park will now be accepting the PPE Medical Eligibility Form and the Health History Update Forms either in person or through email.    The forms can be scanned and emailed to [email protected] or to our new school nurse Ms. Baker at [email protected].  When submitting health documents by email,  please attach the forms to a blank email and include the students first name, last name, and sport they are playing in the the subject line of the email.
