Winter Sports Registration for Grades 7-12


Winter Sports Registration:  Online registration will open for fall sports on Tuesday October 19th.

Registration for the following sports will be open:
High School: Basketball, Track/Field, Cheerleading, Bowling, Wrestling

Middle School: Basketball, Wrestling

Please follow the registration guidelines located below. Health related paperwork should be submitted directly to the school nurse.

Below are the instructions for MP sports registration. Students must submit appropriate paperwork to the nurse prior to participation.  

Attention Parents and Student-Athletes:
DO NOT EMAIL PAPERWORK. PAPERWORK MUST BE SUBMITTED TO NURSE.

Please read thoroughly for info on MP Sports Registration.

Please follow the steps below.  Doing so will ensure that your child is cleared to participate in time for the sports season.

Step 1:  Submit the electronic forms online.

The online submission website is open.

All athletes must submit online forms first, BEFORE submitting physicals to the Nurse.

To submit forms electronically, click on the link: http://mphs.powermediallc.org

Next, click on "Sports Participation Packet" in the top right corner.

Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.

Finally, follow the prompts to complete all the necessary forms electronically.

Step 2: Submit a New Physical or Health History Update.

Students must have first completed their online forms before submitting Hard-Copy forms!

Once online forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

Dear Parents, Students and Midland Park Community,


We are excited for the start of the 2021-22  sports season at Midland Park Jr./Sr. High School!  At this time we wanted to share with you important guidelines from the governor’s executive orders and our district. With health and safety being the first priority, the following guidelines will be in place for the 2021-2022 sports season. Please understand that these guidelines are subject to change based on information from athletic, health and state agencies. In that event, you will be notified during the course of the season. 

Outdoor Sporting Events

Spectators may attend outdoor events to home games and matches. Face coverings are optional. Spectators are asked to practice social distancing.Sidelines should be limited to active participants and personnel that are essential to the event.

Portable bathrooms are located behind the stadium bleachers.

Indoor Sporting Events

Spectators must wear face coverings. Spectators are asked to practice social distancing while inside the school/gym. 
Sidelines should be limited to active participants and personnel that are essential to the event.


Failure to comply with these rules may result in possible removal from the grounds at home and/or away events. Please contact me if you have any concerns, questions or reasons for possible exemptions from the guidelines above. Many of these guidelines are new to all of us and we ask for everyone’s cooperation so that we may enjoy our sports season and provide the best possible environment for our students, families and spectators.
Spectator guidelines for away games/matches will be determined by each individual school district.
