Please read thoroughly for info on the Midland Park Athletics Registration process and follow the steps below. Doing so will ensure that your child is cleared to participate in time for the start of each season.  Students must submit appropriate paperwork to the nurse prior to participation.

*HARD COPIES OF ALL HEALTH PAPERWORK MUST BE SUBMITTED TO THE NURSE *



Step 1:  Submit the electronic forms online.

  • The online submission website is open.
  •  All athletes must submit online forms first, BEFORE submitting physicals to the Nurse.
  • To submit forms electronically, click on the link: http://mphs.powermediallc.org
  • Next, click on "Sports Participation Packet" in the top right corner.
  • Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.
  • Finally, follow the prompts to complete all the necessary forms electronically.

Step 2: Submit a New Physical or Health History Update.

  • Students must have first completed their online forms before submitting Hard-Copy forms!
  • Once online forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.
  • The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in. State Athletic Physical Form
  • The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed. Health History Update Form

Weather/Lightning: Midland Park High School is equipped with a lightning detection system. Additionally, from the NJSIAA, once lightning has been recognized or thunder heard, by an official, a coach, the host site management personnel, or by a lightning detection system, the game must be suspended immediately with all players, coaches, spectators, and officials directed to appropriate shelters. athletes, coaches and personnel will report inside of the high school. It is mandatory to wait at least 30 minutes after the last flash of lightning is witnessed or thunder is heard. Any subsequent lightning or thunder after the beginning of the 30-minute count must reset the clock and another count must begin. Once the contest has been suspended, the 30-minute mandatory suspension in play is in effect. If the lightning detection system gives an “all clear signal” prior to the end of the 30-minute suspension time, the contest shall not be resumed until the 30-minute suspension time limit has elapsed, per the NJSIAA and NFHS policy. However, if a member school has a Board policy that states no play/no activity may resume until the lightning detection system gives the “all clear signal” even though the 30-minute suspension time has elapsed per NJSIAA/NFHS rule, that Board policy shall supersede NJSIAA/NFHS policy.
