FALL Sports Registration for Grades 7-12

Fall Sports Registration:  Online registration will open for fall sports on Thursday, July 1st.

Registration for the following sports will be open:
High School: Girls Tennis, Girls Soccer, Boys Soccer, Cross Country, Football, Cheerleading, Girls Volleyball

Middle School: Girls Soccer, Boys Soccer, Girls Volleyball  and Cross Country

Please follow the registration guidelines located below. For the fall season health related paperwork should be submitted directly to the school nurse during the below dates/times.

Health/athletics paperwork submission dates:

July 14:  12:30-1:30

July 21:  12:30-1:30

August 3:  9:00-2:00

August 4:  9:00-2:00

August 5:  9:00-2:00

Organized Summer Activities: Fall, winter and spring student-athletes wishing to participate in team organized activities at Midland Park High School during the summer (that are prior to the first official day of practice) must register online under SUMMER SPORTS REGISTRATION.

Additionally, those student-athletes must submit a HEALTH HISTORY UPDATE FORM to the school nurse prior to June 22nd.


Below are the instructions for MP sports registration. Students must submit appropriate paperwork to the nurse prior to participation.

Attention Parents and Student-Athletes:

Please read thoroughly for info on MP Sports Registration.

Please follow the steps below.  Doing so will ensure that your child is cleared to participate in time for the sports season.

Step 1:  Submit the electronic forms online.

The online submission website is open.

All athletes must submit online forms first, BEFORE submitting physicals to the Nurse.

To submit forms electronically, click on the link: http://mphs.powermediallc.org

Next, click on "Sports Participation Packet" in the top right corner.

Then, choose "new user" if you've never completed online forms, or "current user" if you completed forms online previously.

Finally, follow the prompts to complete all the necessary forms electronically.

Step 2: Submit a New Physical or Health History Update.

Students must have first completed their online forms before submitting Hard-Copy forms!

Once online forms are submitted, Students/Parents must still hand in a hard copy of either the State mandated Physical Form or the Health History Update to the school nurse.

The Athletic Physical Form (found in the link below) must be newly completed every year.  The Athletic Physical Form is good for 365 days.  When a NEW physical is submitted to the nurse, there is no need for a Health History Update to be handed in.

The Health History Update Form (found in the link below) must be completed EVERY ATHLETIC SEASON when a student-athlete's physical form is passed 60 days of being completed.

     *The NJSIAA has recently published its most up-to-date version of the Return to Play Plan '20-'21.  Please note that this document is an ever changing document, and will be continuously updated.

*The NJSIAA has also asked member schools to share the video NJSIAA Return to Play: Q & A with Dr. Damian Martins.
